1. Open up your Microsoft Word document and save the document as a PDF
    1. Option 1 - Click Save as Adobe PDF
    2. Option 2 - click PDF from the File Type drop-down menu



  2. Open Adobe Acrobat PDF program, then select File > Open and select the PDFthat you just created.
    1. Note: Be sure to open the Adobe Pro program, not the Adobe Reader program.




  3. Click on the Tools tab on top, then click on Prepare Form


  4. Click the Start button to convert it into a fillable Form



  5. Make any edits to the Form if needed by using the Tool bar on top
    1. Delete text boxes and replace with a checkbox.  



  6. Click the Preview button to review & test form.




  7. Once you are all done, Save the PDF
